

Baby2Home (B2H) is a digital health intervention designed to bridge the resultant gaps in obstetrics and pediatrics healthcare services for new families over the first year of life. The B2H app incorporates features designed to support both parent wellness and infant care. These features cover essential postnatal care practices, including, but not limited to, tracking mechanisms for feeding, sleeping patterns, growth milestones, diaper changes, and vaccination schedules. Moreover, the app engages parents with educational content on infant developmental milestones, conducts monthly mental health assessments, and supports adherence to preventive healthcare schedules for both the parents and their newborns.
The postpartum period represents a critical period for both birthing and non-birthing parents. Parents navigating this period are often faced with challenges such as exacerbated mental health conditions, and the complex task of adapting to new caregiving roles. Despite the presence of Collaborative Care Models (CCMs) as a strategic framework to address these multifaceted challenges of this period, the current landscape of postpartum care is fragmented. This fragmentation leads to a lack of continuity of care, failing to address the holistic needs of new parents during the postpartum period. Baby2Home (B2H) is a digital intervention rooted in the CCM, specifically designed to support parents through their transitions into parenthood by addressing their physical, emotional, and psychosocial needs. This intervention seeks to close the gaps left by traditional care models by providing continuous, organized, and accessible support throughout the postpartum period.

The Baby Tracking features supports parents by providing intuitive care trackers designed to boost parenting confidence and competence. Parents can track feeding, pumping, sleep, diaper changes, growth, and important health information such as vaccinations and medications in one centralized place. A built-in notes feature allows parents to create and organize questions for their healthcare provider, helping them feel more prepared for appointments. The shared tracking functionality enables partners to collaborate seamlessly, making it easier to stay aligned and parent together.

The Parent feature offers parents access to over 250 curated articles covering baby care, developmental milestones, parenting guidance, common challenges faced by new parents, self-care strategies, and physical and mental health wellness resources. The library also delivers anticipatory, time-based educational content including articles and videos, tailored to the baby’s age and each family’s unique parenting journey, ensuring parents receive relevant support exactly when they need it.

The Mental Health Screening and Support feature provides monthly assessments using standardized tools such as PHQ-9, PSS, and GAD-7 to help parents monitor their emotional well-being over time. Built-in reminders and notifications encourage timely completion of screenings, while score progression reports allow users to track changes and trends in their mental health. Based on screening results, parents gain access to tailored mental health modules, including Stress Management and Resiliency Training (SMART) for stress, Behavioral Activation (BA) for depression, and Daily Boost activities—short, self-care practices designed to build healthy habits and promote overall mental wellness.

The Care Manager Chat feature provides parents with access to live chat support for both mental health and infant care needs, offering timely guidance when questions or concerns arise. Care managers continuously monitor screening results through a dedicated dashboard, enabling proactive risk assessment and triage. They share relevant mental health and infant care resources, help parents create personalized care plans, and assist in finding and connecting with providers covered by their insurance. Ongoing follow-ups ensure continuity of care and support relapse prevention planning, helping parents feel supported throughout their care journey.

The Care Manager Dashboard is designed to give care managers a centralized view of parent progress, allowing them to track engagement, review mental health screening results, and monitor behavioral health trends over time. The dashboard includes a secure chat feature for direct communication with parents, making it easy to share educational content, resources, and personalized support. Furthermore, the centralized dashboard also supports risk assessment and triage by automatically highlighting parents with elevated screening scores who may need immediate attention. It enables care managers to collaborate with parents on creating personalized care plans and supports ongoing follow-up to ensure continuity of care and relapse prevention planning, helping families receive timely and coordinated support.
